It mainly adds new maps, game modes, perks, weapons and much more for this year's version. In order to use the new features within the game, you will need to download the latest Windows update.

I dont think mine will work for you as it is in french, but im sure we can find this here. Manual patching does not work if the game is on steam. Right click on the game and select Properties. Once that is done restart your PC and restart Steam. If it is going to patch COD4 it should start auto patching as soon as steam is loaded up.
Hope that works. There is no need to reinstall any game on Steam unless you tried to manually install Retail patches. Some slacky on their end did not know that there was a version 1.
